I've never gotten a spam comment, but every time I list my actual email on my sidebar instead of just an envelope that needs to be clicked, I get weird spam email. It's too bad you have to go to that extra trouble - tsk!

This is why I use wordpress on my personal blog. My other blogs are too obscure for most people to notice. ;^)
And yes, I require comment approval for all new commentors. Anyone with an approved comment can comment freely. I use Akismet and WP-spamfree which keep all known spammers away, and only get spam as new computers are compromised (right now by the conficker worm) and add to the problem. Please do your part and run your virus checking software frequently to keep these worms and viruses out of your computers, everyone.
I have not had much spam, and I have comment moderator so I can vet all comments... and delete any nasty ones! It's a good idea.
Last week I had 15 spam comments from the same a-hole. Pissed me off dude.
